What is the Dynamic Support Register?
- The Dynamic Support Register is designed to help professionals across organisations, when working with individuals who have a diagnosis of Learning Disability or Autism.
- The Dynamic Support Register aims to identify risk.
- The network then works together to come up with a plan to help mitigate that risk in the medium to long-term.
- We will work together to identify provision within community services.
- The network will look to identify alternatives to hospital admission.
- Support timely discharge for individuals who are currently in mental health hospital.
- The Dynamic Support Register is NOT a crisis service. If you, or someone you know, is currently in crisis please contact your local crisis services.
Why do we need a Dynamic Support Register?
Local NHS Services need to understand the needs of individuals with a Learning Disability & Autism who might be at risk of admission to hospital. This ensures:
- that this group of people receive the correct support
- the Integrated Care Board (ICB) is required to maintain a register of individuals with a learning disability or autism who:
- They have a mental health condition where they have been admitted to hospital or likely to need admission to hospital.
- Display behaviours or likely to display behaviours where their home life or placement is at risk of breaking down.
North Yorkshire and York Dynamic Support Register
- The purpose of the DSR is to ensure that an integrated approach between health, social care and education is in place across North Yorkshire and York, for the risk management of individuals with learning disabilities, autism or other mental health issues.
- The overall purpose of the group is to ensure robust care packages are in place for hospital discharge, minimise the need for hospital admission, reduce health inequalities and improve outcomes for individuals with learning disabilities, autism or other mental health issues.
- The group brings together professionals with responsibility for commissioning, procuring and delivering services to individuals who are at risk of admission or who are inpatients in specialist hospitals.
- The professional who has referred an individual into the DSR will provide feedback as to the plan and outcome from the meeting.
- Advice and referrals to the services that can provide the support you may need will be made by the lead professional
- Communication will be through the lead professional who referred you to the DSR.
- If you referred yourself, we would require you to also be accepted by a service within our network to enable the prompt feedback of information to you.
