Our Health and Care Partnership approach

Our community’s health and care system has a rich history of teamwork and cooperation, benefiting local residents for years. Our partnership ensures that various organizations work closely together to address local needs while still fulfilling their individual roles.

We’re evolving towards a more comprehensive way of doing things, bringing together mental, physical, and social care for people of all ages. By different organisations working together seamlessly, we aim to achieve better outcomes for everyone in our community. This involves collaborating on a broader scale to address health inequalities and put the needs of our residents first.

We’re committed to reducing disparities in health outcomes and ensuring that everyone receives the care they deserve. Our focus is on creating “Accountable Care Teams” that span different population groups, using a holistic approach to improve overall health and well-being.

North East Lincolnshire is home to around 156,940 people, mostly living in coastal and urban areas. In our community, there are differences in opportunities and resources, with 37.7% of the population living in the 20% most deprived areas.
