Centre of Excellence for Frailty

Across our region, there are more people aged over 65 than the national average. This ageing population will continue to grow, and increasing numbers are living with frailty. We want to ensure people are empowered to live healthy and independent lives for as long as possible. For people living with frailty, this means early diagnosis and targeted support, taking a patient-centred approach to understand their preferences and priorities.

The Centre of Excellence for Frailty supports the health and care of older residents across Humber and North Yorkshire. Launched in 2024, it builds on the pioneering work of providers in the region and facilitates the sharing of clinical expertise and best practices to transform the lives of older people.

By encouraging a standardised approach to the prevention, identification and management of frailty, the Centre can ensure older people receive consistent, high-quality care and treatment – regardless of where they live.

We understand that our region is made up of a range of communities, each with unique challenges, and solutions must meet their needs. 

Working collectively, the Humber and North Yorkshire Health and Care Partnership is working to put in place the building blocks for a healthier future.

Our guiding principles

Ten principles have been developed for the Centre of Excellence which are fundamental to delivering an integrated care model for people living with frailty. They will ensure that all the needs of all our communities are considered to create a consistent approach to the health and wellbeing of older people.

Our ambition is to enable clinical leadership and expertise to be shared across Humber and North Yorkshire through training and peer support. By acting on these principles, we can establish a workforce focused on frailty to deliver improved care across Humber and North Yorkshire.

In Selby, health and care professionals are working together to provide valuable resources and support for the community. The Ageing Well Health Fair gives people the opportunity to access a variety of services aimed at promoting healthy living.

We want to create more efficient hospital-based services for people who need them, making the best use of the resources and workforce across our system to plan and deliver hospital care.

We want to help more people to survive cancer and support people in our region to live well with and beyond cancer.

We aim to ensure that people in our region are able to access advice, care and support in an urgent or emergency situation.

Working together to improve elective (planned) care locally is a key priority for the Humber and North Yorkshire Health and Care Partnership.

We want to create more efficient hospital-based services for people who need them, making the best use of the resources and workforce across our system to plan and deliver hospital care.

Climate change poses the most significant long-term threat to our health, not to mention our planet. The Greener NHS programme will work with staff, hospitals and our partners.
