The York Health and Care Partnership includes partners in health and care organisations from across York who are all working together to improve the health of people in York.

Humber and North Yorkshire Health and Care Partnership is one of 42 Integrated Care Systems (ICSs) which cover England to meet health and care needs across an area, coordinate services and plan in a way that improves population health and reduces inequalities between different groups. The Partnership comprises of NHS organisations, local councils, health and care providers and voluntary, community and social enterprise (VCSE) organisations as shown in the table below.

Our partners include Humber and North Yorkshire Integrated Care Board, five acute trusts, three mental health trusts, six local authorities, two ambulance trusts and four community interest/not for profit organisations.

York sits as one of six ‘Places’. In York, our Place Partnership is called the York Health and Care Partnership (YHCP). The YHCP brings together partners across health and care to deliver improvements in experience and outcomes for people living in York.  A list of our partners is below.
