Population Health Hub

The Population Health Hub (PHH) is a multi-organisation group which brings together colleagues from health, public health, and business intelligence to enable, analyse and undertake population health management approaches.

Our work with partners in care focusses on collaboration between health and social care to support staff and improve quality outcomes.

York Health and Wellbeing Board

The York Health and Wellbeing Strategy sets a clear vision that over the next decade ‘York will be healthier, and that health will be fairer’.

Our work with populations in York goes beyond diagnosing and treating illness, putting more emphasis on improving well-being and preventing ill health.

We are part of the wider Humber and North Yorkshire Health and Care Partnership which is an ‘Integrated Care System’ working to improve health and wellbeing.
